Of the several choices you will have to make once you decide to pursue a breast augmentation, where the implants are placed is one of the most important, and one that you will want an experienced plastic surgeon to advise you about.

There are two options: subglandular (above the pectoral muscle) and subpectoral (under the pectoral muscle). Each has its own pros and cons, and it may be that your aesthetic goals, body type, or lifestyle may make the decision an easy one. Here is a brief discussion of each placement option.

Above the pectoral muscle (subglandular) – This is the most popular choice, as it has several advantages. The surgery is less involved and allows for a much quicker recovery time and less post-operative pain and discomfort. Surgeons have much more control over the contour of the breasts with a subglandular placement. Depending on the volume and position of your natural breast tissue, subglandular placement may be your only good option. Generally speaking, subglandular placement provides the most natural look and feel of the two options.

Subglandular placement may not be the best option for women with thin skin or little to no natural breast tissue. Ripples caused by the implant tend to be more pronounced in these cases. Subglandular placement also puts you at a greater risk of capsular contracture and typically complicates mammography and breast cancer screening. Subglandular placements are, however, far easier to remove, replace, or repair should they ever puncture or become damaged.

Below the pectoral muscle (subpectoral) – Women with little or no natural breast tissue often choose subpectoral placement. This provides a more natural look and decreases the chance any rippling that might otherwise occur with a subglandular placement. Subglandular is a good option for women who want less-pronounced cleavage and a more natural slope to their breasts. You may have a longer recovery time with subpectoral placement.
